Medical case management means having someone on your side to help you understand your healthcare, coordinate appointments, connect to resources, and guide you through difficult situations. We help make healthcare easier and less overwhelming.

No. We are not a clinic or medical provider. We work alongside your doctors and healthcare team to help coordinate your care and connect you to support services.

Services may be available for Medi-Cal members who: Have chronic health conditions Visit the ER or hospital often Need help managing healthcare Are pregnant with high-risk needs Have mental health concerns Need support with housing or community resources Have complex medical or social needs If you are unsure if you qualify, contact us and we can help check.

No. Services are free for eligible Medi-Cal members. Services may be covered through your Medi-Cal health plan.

We currently work with members who have: Kaiser Permanente Molina Health Net Blue Shield Promise

No. We do not provide housing or emergency shelter directly. However, we may help eligible members connect to housing programs and stable housing resources by: Assisting with housing applications Connecting members to housing navigation programs Referring to shelters and community agencies Helping complete Coordinated Entry System (CES) applications Connecting members to Community Supports housing services through Medi-Cal when eligible

No. We are not a food pantry. We help connect members to: Local food pantries Meal programs CalFresh/SNAP resources Community food assistance Medically tailored meals or nutrition benefits available through eligible Medi-Cal health plans

No. We do not directly drive members to appointments. However, we may help: Schedule transportation through your health plan Connect you to transportation assistance programs Help arrange rides through local services such as: Access Services Dial-A-Ride Metro Non-emergency medical transportation programs

Yes. We may help connect you to programs and application assistance for: IHSS CalFresh SSI/SSDI Medi-Cal Community resources

Yes. In some situations, our staff may accompany members to appointments to provide support and help with care coordination.

Enhanced Care Management is a Medi-Cal benefit that provides extra support for members with complex medical and social needs. It focuses on whole-person care by helping members coordinate healthcare, community services, and wellness support.

Community Supports are services that help address everyday challenges that affect health, such as housing navigation, food support, and other social needs.
