Real-Life Examples of How We Help
Cancer Care Support
A member recently diagnosed with breast cancer felt overwhelmed managing appointments, treatment, and paperwork. Empower Health helped coordinate oncology appointments, explain treatment instructions, arrange transportation through the health plan, and connect the member to cancer support groups, a free wig program, local food pantry resources, and IHSS application assistance so she could focus on her recovery and daily well-being.
Housing & Medical Support
A pregnant member experiencing homelessness needed help managing prenatal care while searching for stable housing. Empower Health helped coordinate medical appointments, connect her to housing navigation services, assist with applications, and provide ongoing support throughout her pregnancy.
Chronic Illness & Hospital Follow-Up
A member with diabetes and high blood pressure was frequently visiting the emergency room because they struggled to manage medications and follow-up care. Empower Health helped schedule doctor appointments, provide health education, coordinate transportation, and connect the member to food and wellness resources to improve their health and reduce hospital visits.
Child with Complex Medical Needs
A child with asthma was missing school often due to frequent medical issues and difficulty accessing follow-up care. Empower Health helped the family coordinate specialist appointments, provide education on medications and asthma management, connect the family to a free air purifier and community support resources.
Foster Youth Transition Support
A foster youth aging into adulthood needed help managing healthcare, benefits, and independent living resources. Empower Health helped connect the member to primary care, mental health services, housing resources, educational support, and community programs while helping them learn how to navigate healthcare and daily responsibilities independently.
